Unhappy getting several codes - need some help running down fixes

I have a 2000 Formula M6. It's basically stock except Borla cat-back, SLP Y-pipe, airbox lid, K&N filter & smooth flow bellows. I also have the coolant by-passing the throttle body. The MAF is stock and has not been modified.

The car has abut 94K miles on it. Problem is it runs rough right around 2500 RPM regardless of gear at less than WOT. It smooths out around 3000 rpm, but still feels sluggish. It almost feels like I get out of the gas when I hit 2500 rpm. When I run it wide open it runs smoother but still feels a little sluggish.

I started out with the following codes: P0171, P0174 and P0430.

I've attempted to run down vacuum leaks and couldn't find anything. The wires on my MAF look clean. So I installed new O2 sensors (Bosch 13444) on both banks in front of the cats. I had also put on a new fuel filter about 2K miles ago. Fuel pressure seems fine, I've measured it at about 60 psi at idle.

After I put in the new O2 sensors I got the following codes: P0137, P0157, P0171 & P0174. So I am guessing I missed a vacuum leak based on P0171 & P0174 and I need to replace the back 2 O2 sensors based on P0137 & P0157. Is it possible that bad O2 sensors would cause the P0430 code or is my cat just shot?
