Default Hesitation when shifting

Ive had this problem since I put headers on the car, but now the combination of my cam and the cold air temps is making this problem more severe.

Ill be driving, taking off from a light, make the 1-2 shift fine, continuing with ~15% throttle, make the 2-3 shift and BAM the car bogs and I lunge forward toward the dash for a fraction of a second before the motor comes back to life. It usually happens when the car was cold started and is just about to reach operating temp (but is already in closed loop), but it also happens occasionally even when the car if fully warmed and has been driving for a while. What should I look at in my tune? This is becoming very inconvenient and quite embarassing when i have passengers.
