Fuel System Question!!

Fuel System Question!! My car has been throwing the SES light left and right.
Everytime I Atap it, I see that STrem Fuel Trim Bank 1
and STerm Fuel Trim Bank 2
The last thing that was installed was a ls6 intake. The intake was tapped and unlike the huge egr tube that goes down in the intake, we used something about half the size. Well ever since I have been getting the codes. Could it be that the tap is loose and not fitted air tight causing a leak and leading to my problem?? Or could it be that the smaller tap fitting is throwing the codes??
Any help would greatly be apperciated!
