Default LC-1 error 1 "heater circuit shorted" on install?

Someone please help me with this install. I have wired everything up to use the LC-1 as a narrowband and wideband but I'm just trying to get it to work first. I had all the grounds (blue, white, and green) connected to what I thought was the ground wire (pin A or B) on my O2 sensor extension wire but there was no power going to the sensor. I then disconnected all the grounds from the O2 extension and grounded everything to a chassis ground (a bolt under my dash where other electronics are grounded). Now the LED at least comes on so I know it's getting power. It warms up for the 30 seconds and then I get 1 flash repeating every 2 seconds which is "heater circuit shorted" error. The manual suggests repair/replace the cable or the sensor. Everything is brand new so I can't see having a short somewhere. Could I have just wired it wrong somehow?
TIA

When I put mine in I had the doors, trunk, and hood open long enough that my battery drained to 11.2 volts. I got that 1 flash error as well, but when I charged the battery, it went away.

Is the sensor still functioning even with this error? I don't want to start the car up w/o the sensor working...I heard that can kill the sensor.
