Passenger side pre-cat O2 sensor problems.
Installing headers that place 02s at the wrong distance from heads, thus exhaust
temps are cooler and 02 is reporting rich all the time and never switching to lean.
Incorrect placement of O2 causing its wires to be cut and spliced. O2s have special
wires where the 02 gets is reference air via an airchannel in the wire, loosing
that air supply would cause 02 to not function correctly.
Ill tune, causing AFR to be way out of calibration causing 02 to always be at 1 voltage level.
Blown fuse to 02
Hardware logic of PCM for 02 was damaged.
Bad Cat or exhaust leak.
