Default Cold blooded Vortec 454...

I know this isn't really a truck place, but I was already here and thought I'd throw this out and see if somebody might know what's causing it.

I have a 99 K3500 Chevy truck w a 454/5 sp. When I first start it, it acts like an old carbed motor with no choke. LOL
Actually to be more specific it starts right up and idles fine, but when I give it throttle it stumbles and almost dies. If I play with the throttle enough to get it past 2000rpm or so it picks up and runs fine, or if I let it return to idle it idles fine again.
After the engine warms up this problem disappears and it runs fine. (Colder weather makes it worse).

It doesn't seem to set any codes. I'm assuming because it runs fine when warmed up.

Any ideas?

Hmm. Never considered that.
I was thinking IAT sensor or coolant temp sensor the whole time, but I'll look into the egr.
