just google " pantherlifts "
I paid 1495.00 for 9k lift a 11k was about 400. more pick up in Cleburne, Tx , I think shipping to me would be about 250-300
need 144" to top if your slab is around 4" should be ok, very easy to install 2-3 men 4-5 hours, happy with lift useing 3 years

Sounds like a good lift... I couldn't find on the website anywhere if that lift is assymetrical or not though.
