Gouged Throwout Bearing

I swapped out a spec 4 and slave for a tex dual disk and new slave. The spec wasn't holding at all anymore. When I pulled out the slave I saw this:

Is this normal?

you probably had to large a shim or you like to ride the clutch/hold it in at stop lighta
