Do these things actually pick you up HP by fooling the computer into leaning the engine out?

Yep, you can pick up HP with one of these. It is kind of the poor mans way of tuning.

You can pick up a decent amount of RWHP by using one of these, assuming you're running pretty rich. I love mine. Just don't buy it, throw it on and start dialing. You need datalogging software to monitor what changes you're making. Search the site for posts where people explained the best way to tune a MAFT and save yourself a little trouble.
