Default Th400 oil pump keeps breaking

Ok fellas, PLEASE HELP and chime in if you can. I can really use the advise.

I have a th400 RMVB from FTI and a 9 inch converter from FTI behind a lsx 454 (true meziere flexplate #fp319) with a callies compstar 4340 4.125 crankshaft.

MY ISSUE: This is the 2nd trans oil pump that has gone out on me. I am only getting about 70 miles out of these oil pumps. both times we checked the pressure after the install and had about 200 psi trans pressure. What can cause this to happen?

When i ordered my converter i had them set it up for a LS flywheel/motor. But i still had to hone my flywheel holes a tad bit to get the converter to line up with the flywheel holes. is this ok?

I am using the 1 inch spacer with a m6 crossmeber, is this ok? or is the angle of the trans causing an issue? any help is appreciated.

If it is scarring the outside of the outer gear and the pump wall, most likely the transmission is not centered to the crank.

Check the dowel pin bores in the case, the dowel pins on the block (good idea to put longer ones in), and have the converter drive hub runout checked.
