You say it "won't shift into 2nd or 4th manually"; does it shift automatically?
Please describe exactly what happens when you start in [OD] and accelerate.
Glad you have read the related threads and have already checked the servo end play.
How many miles on the truck?
I suspect the servo is relatively easy to remove on your truck (not on a F-body) and replacing the seals would be my next step. With 1/8" end play your band should be fine, and so far this does not sound like the trans needs a rebuild.
I can't find my posts on how to remove the servo right now, but the basic steps are:
1. Hit the cover with a mallet to loosen it. (You probably already did this to measure end play)
2. Remove the large wire ring holding the cover
3. Pull out on the cover until the blue o-ring is visible.
4. Grab the blue o-ring and cut it with wire cutters.
5. Pull out the blue o-ring.
6. The entire servo comes out easily then.

OK, it is clear your band is not engaging. I assume you have Reverse. (This ensures your sun shell is OK; 2003+ sun shell failures are very rare.)
The o-ring and seal kit for a servo is very cheap; maybe $15.
If you are sure about the end play, skip the extender. Also extenders can fail, it is better to get a long pin, either Sonnax or GM.
