no 2nd or 4th
Are you sure you don't have 4th either? With enough speed will it shift from 1st to 3rd and then nothing more?
Curious, how did you air test the servo? I know there is a 4th servo drain hole that can be used for testing, but don't know of any easy test for the 2nd servo. (Other than a test plate which replaces the VB, but few have that.)

The torque converter affects all gears and even reverse equally as it has no idea (must be dumb) what gear you are in.
What band did you reinstall? wide/stock?
You should not need 2 "Gold Spacers" Most 4L60E's come with a 2 groove "Mid-length" apply pin, With a GOOD reverse input drum.....You should only need 1 spacer.
Did you pull the Input Drum & Reverse Input Drum & Check the Reverse Input Drum surface? Good time to at least check the 3-4 clutches as well.
This "replacing a Friction" **** rarely works out.
