Default Impressive Front Planet Failure

Customer brought in their trans with a loss of all forward gears. Seemingly randomly one day.
I was expecting to tear it apart to find a cut teflon or something similar exhausting all forward clutch oil...I was surprised to find this!!

Ripped the spline hub right out of the carrier!!

Just a Heads/Cam 6.0 in a full size pickup.

Looks like a needle bearing failed and locked it up.
No idea why the splines ripped out like that instead of chewing a planet gear or something though.

This is the first time I have ever seen this. Probably just a fluke. Look at the output shaft and check for any twisting of the splines. Did anything happen out of the ordinary here just before this happened?

Nothing out of the ordinary that I have heard from the customer.

Output looks fine. Couple scratches from having to press it out, but that's it.
