4L60E - Two gears at once

4L60E - Two gears at once If I select first gear on my box it feels as tho it is also engaging another gear. The car will not readily creep on idle like it does in drive.
It is also shifting into second well before the required speed in drive. It changes at 3500 rpm when it should be closer to 6000rpm. I have fiddled with the change at speeds for WOT and Part Throttle but it makes no difference. It is also hanging in second gear and not changing to third.
Car has a stock stall and B&M shiftkit.
Any ideas as it is driving me nuts

The 1=>2 accumulator piston was still the stock plastic one and had broken into three pieces held together by the o-ring. Was replaced with a metal one and so far so good.
