even after warming up, at 165ºF, the narrow-band O2 sensors still read rich.

I'd start by checking your IAT sensor. If the IAT reports colder temps than the actual air temperature, it will cause the motor to run rich for sure. Its a simple thing, but I'd start there.
exhaust manifolds are homemade, I couldn't find any production manifolds or aftermarket headers to fit the bastard catera, so I made some. I'll try to post some pics next week.

You can also turn the key on, and feel the o2 sensor, to see if it gets warm. Again, do this with completely cold engine, so you can tell heaters are working.
