I finally solved the problem. All the fuel injectors were blocked. I think the engine had been sitting around for a while, and I think the fuel in the injectors had gone off.
I put a current one the injectors and gave them a blasting with compressed air, which sorted the problem. I am currently getting them professionally cleaned and balanced.

Congratulations... how did you finally narrow it down? Perhaps you had a really generous friend who let you dismantle his engine to try your fuel rail on it and would like to publicly thank him?
