What I did was bend some coat hangers to roughly the shape i needed and went up to the parts store and compared them with the wall of hoses. I am not near my car right now otherwise I would give you the part numbers I used.

A body, chevelle and such
I found some numbers on here with a search but they weren't as "definate" as the ones I seem to remember reading about.
One thing I'm curious about is that I'm pretty sure the outlet on the ls1 water pump seems smaller than the radiator

I just used a 98' Z28 Camaro Lower hose and with a little trim it fit ok..! don't recall that number but it was Goodyear hose..
The upper I also used a Goodyear, #26347x but added another hose of the same size in just a 90 Elbow to push the hose away from my CAI tubing. Else it would have worked without the 2nd piece added. I think it was also a 98' Z28 Camaro hose..?

Stock Tahoe upper and lowers fit my car just fine. I am using the truck water pump though... Maybe cutting a upper truck hose? Lower should work. Parts store, ask for a lower rad hose for a 2004 Chev 1500

Here's my setup:
Upper Hose - stock F-Body from P/S cooler to motor, 90 degree hose
from GATES (unsure of P/N) from P/S cooler to radiator
& trimmed to fit.
Lower Hose - Gates hose P/N 22437 (trimmed slightly)
Heater Bypass Hose - Gates hose P/N 42570187

Visualize your set-up from radiator to engine. Make a few measurements & write them down. Go down to NAPA or other, tell them you're doing a swap and need to pick out hoses for your application -- then go cross-eyed looking at the 100's of hoses hanging on their wall. We used several different hoses, spliced together for our swap. Works good!
