how to go after city/county/etc??

A few nights ago I was leaving chipotle in Altamonte Springs and I was taking the little back road behind it to leave.
Just as I approached the stop sign (NOT on chipotle property, the road behind it that connects all of the businesses/restaurants) I saw what APPEARED to be a freshly paved pothole...
Turns out it was an extremely deep pothole that was so deep it looked black.
I was only going about 5mph because I was about to stop and the right front fell into the pothole and it hit so hard that my car shut off.
I started the car, and drove away EXTREMELY angry.
After pulling into my garage, I checked out the damage. The wheel is a little beat up, the tire is cut for about 8" at the bead where the wheel cut into it, and the fender is bowed out (just like when someone jacks the car up incorrectly, this is where the car landed in the pothole)
the tire holds air, it's just cosmetic, but I don't trust it at all, and the wheel is probably bent.
sorry for such a long *** story, but I'm extremely frustrated and am wondering if anyone here knows the proper steps to take in order to have whoever is in control of maintaining that road take care of this.
I'm assuming I'll need to take some photographs, etc, but I don't know who to contact and what to say.
Any help would be greatly appreciated. thanks!

it can be done i work at nissan in tampa, and i have seen many times bent rims and flat tires caused by pot holes. and the city has paid every time. dont know how to go about it tho. i will try to get more info tom at work
