Just saying, as usually a piston/plug would go first if lean. It's usually an impact of some sort that leads to a dropped/broken valve, or if somehow a collet/retainer came undone allowing the valve to drop
Always nice to be sure of a cause, so it wont happen again. the damage is always horrific. IMO it is the single biggest and worst failure you can ever see in an engine, as it destroys everything !

what do you mean by it's usually an impact of some sort!!??? yes of coarse me and my engine builder/tuner want to know 100% the cause of the damage don't want it to happen again !!! even more so with an rhs block !!!

ie. valve to piston contact, valve to valve contact ( multivalve engines )
Sometimes it can be a valve failure if a 2 piece valve, where the two sections are welded together, but I think most high performance valves used today claim to be 1 piece, so that shouldnt really happen.
Even minor contact from say an over-rev might give a slight bend in the valve but with no real obvious symptoms. Over time this will fatigue and fail
But I just cant see an instance of a lean condition causing a valve failure.
I completely destroyed an engine through slightly lean and too much timing during some mile runs, pistons completely cooked, blew a hole through the head/block interface due to detonation, and heated the valves to the extent they actually turned the flat heads concave ! like upside down umbrellas.
But all valves still remained intact.
