Chasing Gremlins oh my! (DTC P0171,P0174,P0300,P0440,P1416)

Condensed version:
Wife took the care to a lube shop and they claimed we needed a new fuel filter (even though I replaced it 12,000 miles ago.)
A week later the car occasionally stumbles when accelerating from a stop and throws:
P0171 Fuel Trim Lean Bank 1
P0174 Fuel Trim Lean Bank 2
P0440 Evaporative Emission (EVAP) System
I did a visual inspection, checked all fuel/electrical connections, inspected gas cap, cleaned the K&N air filter, Throttle Body, and Inspected the MAF, cleared all DTCs. Can't smell or see any gas leaks.
Three days later:
P0171 Fuel Trim Lean Bank 1
P0174 Fuel Trim Lean Bank 2
P1416 Secondary Air Injection (AIR) System Bank 2
Changed Fuel filter, ran a bottle of "Techron cleaner" through the car. The rubber hose from what I believe is the PCV (large black round thing attached to a hose just to the passenger side side of the Throttle Body) appeared kinked so I fiddled with it, and it's round again. Hooked up a Fuel Pressure gauge at the rails and it's 50 @ 0n, and 61ish @ idle.
Car then threw:
P0171 Fuel Trim Lean Bank 1
P0174 Fuel Trim Lean Bank 2
P0300 Engine Misfire Detected
I cleared the DTCs and logged the trip to work this morning so I can try to look over the logs later, but any insight as to what I'm looking for would be helpful.
Side notes:
01 LS1
68,000 miles
Original Plugs, Wires and O2 sensors.
Only mods are TB coolant bypass, Air lid, K&N filter, smooth Bellows.
I have my 2000 I can barrow parts from to test things on the 2001 if needed.
I was thinking my next steps should be:

- Inspect logs - if I can figure out what I'm looking for
- Swap the 00 & 01 MAFs but I would think I would get a MAF error if it was faulty
- Replace spark plugs and wires
- Check/replace O2 Sensor(s)

Check that whole PCV line for damage, it is known to crack and cause a vacuum leak and lean codes. P0300 is most likely due to the lean condition. If P0440 hasn't come back it was probably the gas cap. P1416 is almost always due to a bad check valve for the air pump hose going into the passenger side exhaust manifold. Take it out and see if you can blow through it in the same direction of airflow (towards the manifold).

Swapped out the PCV today, because upon further inspection the rubber had a hole in it @ the bend between the intake and PCV.
I'll clear the codes this weekend and see what happens, and there was obviously unmetered air getting into the system.
