My buddy's 99 TA developed issues with hitting the rev limiter before it would finally kick from second to third. Not long after that, on a road trip, the car wouldnt shift from second to third at all. He replaced the tranny with another one he already had laying around but I personally think it was the shift solenoids going bad and finally crapping out. Try those first. It is a quick inexpensive fix if it is in fact the problem. While you are in there, do your trans filter at the same time. I hope it is something simple. Keep updating.

X2 on Crystler Kids statement, but what you want to do after you do that, is go back to the person who tuned it, and double check your shift points, just in case he missed them.

shifting
i am a hp tuner... i would say honestly ur tune is alitte tweeked.. make sure ur misfire codes have been been removed, and ur speed and rmp shift points match... i own a 3600 stalled a4 trans am... had the same problem.. i added shift pressure and played with mph vs rpm tables and it took care of it..if ur running a big cam, a4 ls1 cars will throw misfires and cause spratic lock ups and disenaging... just thought i would throw in them factors, seems that i have personally went all the bs and headache to achieve good drivabiltytrack results...

If it only does it occasionally your probably just low on fluid.
The reason I say this is when I installed a Trans temp gauge I forgot to put Teflon tape on the thread for the sensor. I had an extremely small leak and it eventually caused the car to bounce off the limiter once or twice before shifting.
It never read low, however I knew I lost about half a quart and after topping it off I haven't had a problem in 2 years.
My car has a 3500 stall and cam

It is. after i installed 3.90's i have the same problem. played with the tune for a while and finally fixed it after i bumped my rev limiter up to about 6200 and set my shiftt point at 6000. when u intall gear et. the engine can carry through the rpm range much faster, the trans doesnt have enough time to build pressure or isnt use to it. bump your 2-3 limiter up and take it out for a spin
