Default Clutch trying to engage in neutral?

I've had this problem before, and i had the transmission fluid changed and it went away for a little while now its back. What its doing is when its in neutral it will try and catch, it sounds like a slight grinding, its hard to explain. But it will also do it while it is in gear with the clutch all the way to the floor it will jerk the car a little bit. Someone told me that it could be the throw-out bearing, I was wondering if anyone had any ideas as to what it would be.
