DMH Cutout issue

DMH Cutout issue So as some of you know i put my cutout on the other day. Every thing is pretty sweet and i am really happy with it (DMH ???) However the other day i was playing with it. The cutout it self seems to be moving around some. I had it tightened down pretty tight i thought. The problem i am having is that when the cutout turns the slightest bit. It does not sit right in the hole and thus does not allow the door to open all the way 
has any one had this issue and any ideas on what i can do?
