If you have the cutouts open does the exhaust fumes come into the cabin?

Mine didn't. I had the cutout turned to a 45* angle toward the ground. You'll be fine.

Mine didnt either. That was one of my concerns of running it open on the street.

I don't smell any fumes. Mine is turned 45 degrees to the outside of the car as well. This is almost a necessary angle, from what I could tell, to get the cutout as close to the floor as possible due to the flange.
