I don't know who makes one but I will say that he will need to be making some REALLY serious HP to really need more than a 3.5" cuttout (like 900 FWHP or better.)
Having said that, any good welder could fabricate one.

pretty much any good muffler shop could make you up one. its not hard at all, too bad QTP doesnt make a QTEC for 4 inch

87 ta, why would you say that? What size collector are you talking about and how much power are you talking about running? Remember, the collector has to deal with HOT (it being hot makes it have a lower density and thus, a larger volume than cooler air farther down the exhaust tract), merging, and pulsing air. The collector is naturally going to have to be big in comparison to other parts of the exhaust like the cuttout. In other words, dual 3.5" cuttouts placed on down the line would be overkill (not saying that they wouldn't work well) for 3.5" header collectors.

When I have a single 4" exhaust why would I put a 3.5 inch cutout on it?
Because my 4" muffler is restrictive to even 502 hp.
900 FWHP for a single 4" I think not! Dual 3" has proven itself restrictive for even 600 hp cars and 3"x 2 has more area of flow than 4" x 1 - not alot, But it does.
Now if you run a cam with some healthy overlap, Why would you try to ruin that scavaging in the exhaust?
