I think it's a bad/clogged injector. Same thing happened to me. I removed the AIR, replaced header gaskets, no change. On the highway I would give it a little gas and it would buck. Logging with efilive I could see the injector duty cycle was 2x different between left and right banks when it was bucking.
I think what happened is an injector gets clogged, so the O2 thinks that bank is running lean. To compensate it has to richen the entire bank which makes it run like crap.
