Default Barbell question

I am installing a new barbell.
The old barbell pulled right out. I noticed that the old barbell o-ring was showing/not pushed in to the block.
Should the o-ring be pushed inside the block when installed, so that it isn't visible? If so, should the o-ring be lubed, first?

Lube first. Push the bell in until you feel it seat. The back should be nearly flush with the block.

I'm now wondering if the old barbell had been pushed backwards, leading to a lack of oil pressure. As noted, the o-ring was showing.
The new barbell & o-ring has a deeper rear (outside edge 3/16"), compared to the old (1/16"). Could this be a design change?
Once the barbell gets pushed rearward, wouldn't the cover have to be removed to push it back in?
I guess I'm not understanding the mechanics of it.

The rear plate does hold it in place, but if the O-ring fails it can affect oil pressure. Kind of unlikely for it to fail though.
