Default 2002 LS6 Cam bolt working out problem

Back around 2010 or 2011 I did a cam swap for a friend of mine. Car was bone stock never torn apart. I don't remember sayin this but he said that when I took the cam out the front bearing had some copper color to it. At the time the car I believe was either a little over or under 30k. Fast forward to 2014 The timing gear bolts on the camshaft were working out. Not only is this 3 or 4 years later it also is around 10,000 miles later. The cam is a comp cams and the gear and cam bolts were factory. I installed the bolts with red Loctite and torque them down to GM specs. Ive done several swap the same way I did his and his is the only one that did this. The owner takes really good care of the car. He only took the car to the track a few times after the install. He does like to play on the streets but hes not out beating the **** out of the car. I know he takes care of his stuff.
The only reason why we found the problem is when we did the heads swap. The front 2 cylinders walls have some pretty bad scratches that are pretty deep. I also noticed on the front cover it actually looks like at one time the factory pulley could have been rubbing the cover on the driver side. He still has the factory pulley amd it does have a deep grove on the snout that is between the front seal and oil pump. This car drove fine. Had no ses light, the valve train was very quiet for an LS1. The bolts look fine the only thing missing was the pin in the camshaft that slides thru the timing chain. Im pretty sure it was there when I installed it.
Has anyone else had any issues like this? I could see those cam bolts coming back out around 2 to 3 thousand miles if I didn't torque them down right. I cant see it doing it 10000 miles later.

Hmmm, if the pin were missing when you installed the gear, all the stress ends up on the bolts. They would work loose over time.

Did you find the pin in the pan maybe?

Tough one for sure.

That will do it.
