I feel you there. However I regret to inform you, you don't have a fresh air side hooked up. Both of those spots are under vacuum because they are behind the blade, in the manifold. You have to find a way to hook one somewhere in front of the blade so the engine has fresh air. It's weird your TB doesn't have a spot for this.

whats the easiest way to go about this? Surely someone else is using one of these truck throttle bodies? Or would it be easier just getting another TB although I already have my tac module/pedal assembly so looking for another that would work with it all might introduce more complications.

That's why I was looking up things like vented/filtered oil caps as a possible solution. I think you could get away with it as is honestly. I do on mine and I don't have fresh air, but I am also pulling -15 inches of atmosphere out too, or about -7 psi. Depending on the cam you're running, hard to say what your intake will pull. It will be fine at idle, but at RPM you pull a lot less vacuum, when ironically that's when you need more. Fresh air coming in helps push all that out creating a "flow". Most people run some sort of catch can because all of that oil will dump right into your intake.

I wouldn't change the TB. I'd look at much simpler solutions, like maybe a ring with a tube in it right behind the MAF or in front of the TB. So long as that air is coming from somewhere BEFORE the throttle blade, it's positive pressure, not vacuum. Everything behind the blade is vacuum. You could put it farther upstream too. So long as it's filtered air.

Does everyone else avoids the truck dbw throttle bodies? Wish I would’ve just gone dbc since I still have to fab something to get the truck pedal to work in the car. I’ve been searching and can’t even find anything on others using the truck Tb.

I know I have a bit of a different setup here, but you can see the EVAP can hooked up on D/S, the fresh air outlet on TB as well as fresh air hookup on P/S valve cover. Mine are capped off though, because I'm using a vacuum pump.

Well done! My plan is to go with a vacuum pump at some point but for now it's just 10AN lines off each valve cover to a breather can.
