Biggest cam and emissions tests. 
I have a 224/224 - 112 cam and I tuned it to pass NC state emissions 
I just tune it a bit leaner about 2 weeks out, daily drive it to work for those 2 weeks to get all the tests in, and using EFILive to make sure all tests complete.
NC just plugs it into the obd2 and checks that all tests pass, and there is a history and that I have CATS on.
Usually they spend more time looking at all the mods and the car then actually doing the inspection

Then that night after passing, I flash the REAL tune back on.
