i get it too, I cleaned the IAC holes in the TB and it seemed to help it. Might wanna check if your IAC module is about to go out. my car just started this, im going down the list of possibles...IAC, MAF, Fuel Pump, and now new to the list EGR. I had a problem of Hard Cold & Warm starts but after cleaning the idler air holes she cranks right up.

Idle Air Control, its on the Driver side of your TB. Opens and Closes the Two holes inside your TB to control Idle. I hear sometimes they stick due to carbon and gunk. I Cleaned mine with some BG Intake cleaner and it helped a little.

Same problem here. I thought it was due to me adding the Loudmouth II exhaust since I noticed it a couple days after I installed it. It only happens to me when I punch it from an on ramp to the interstate. Normal driving at 45-50 mph doesn't have the hesitation.
It's like I lose all my power for a bit but she slowly accelerates to 70 afterwards from about 45mph.
1st to last gear normal driving, no problems. If I'm in say 3rd or 4th then floor it to get on the interstate, she hesitates.
