It's almost impossible to detect a bad lifter just by eyeballing it. If the roller spins freely and there is no visible damage, it's probably OK. If the bleed hole is blocked, it can sometimes be cleaned with an air nozzle, but I would replace it. If the lifters have lots of miles on them (over 50k miles), and you're planning a cam swap, replace the lifters.
