Default 6.2 Victor Jr PCV Plumbing w/ Accufab 4150

I'm currently getting ready to fire up my build and I need to figure out the PCV routing. It doesn't need to be the best setup currently (I'll get to that eventually). As it sits, both valve covers can have lines to be run from, the valley cover does not. Can I simply just connect the 2 VC lines with a T fitting onto the rear hole in the Vic Jr? A catch can will be coming soon and I don't need a vacuum line out since I am running manual brakes.

Also, I am running an Accufab 4150 and it has 3 rear ports (2 small, 1 larger). What are these for?
