Default Proper dimensions? or = breakage?

Could this alignment have caused side loading of the rocker arm leading to failure? If so how do you correct for this considering even the head studs are not aligned with the valve stem?

The important thing to look at is the alignment and check the pattern when it is all put together. The measurements you have there don't really mean it will be out of alignment. The studs don't have to be aligned with the valve stems.

When moving the rocker arm laterally from side to side it seems like the roller tip was biased towards one side of the valve stem over the other. In fact the roller tip edge almost made it to the center of the valve stem when moving in the biased direction. In other words the roller tip isn't centered on the top of the valve stem. Does this mean I have incorrect alignment? Could adjustable guide plates rectify this problem?

The rollers of the rockers should be centered on the valve stem. Use adjustable plates to get it that way, if needed.

yes, ISKY makes a set which worked well for me to get 'allignment" correct. Mark the top of the valve with a marker pen, lash RR and rotate engine by hand twice. Remove RR and see where witness mark is. If dead nuts center than PR length is right. mark towards exhaust means PR to long, closer to intake means to short. A PR length check tool is $12 and handy.

The pushrod length is correct per the wipe pattern on the valve stem. Its the lateral movement of the rocker tip that is sliding left or right off center. I do believe this can be corrected with the adjustable guide plates. Isky and another freakin $50....
