worst I had was a 400 sbc. engine builder forgot to tighten a rod nut. rod wrapped around the crank, destroying everything but the heads.

Mine is simple: Spend 6 months building a turbo kit; get it tuned for 5lbs; take it out to 30 mph and hear a pop. end of engine after first time pressing down the gas
