Pulled the fuel rail to check injectors and found this:
The good news is that the injectors were not leaking. The bad news is that all of my injectors had these black tips on them except one.
Now what?!

I'd say pull off the intake and see what you can see. Be lucky if it just bounced around in the port, but I'd say very doubtful.
Chances are it went into the head. If you don't want to pull the intake you could always stick a camera down in the injector hole.

I'm pretty sure its plastic and doubt it would really cause any hell. But if you want to be safe i guess tear her down some more but my guess is its gone and you wont find anything.

It's very possible nothing bad happened. But I could also see that getting lodged in a valve and bending it slightly. Compression test on that cylinder wouldn't be a bad idea either.

Back in the day you could buy these at a parts house. Also check it is pretty common for these to come off when you pull it out if the injecter is stuck, however usually the oring stays in the bore if that is the case

Thanks guys!
I ended up calling a fuel injector company and was told that the tip has no mechanical function – it only helps to hold the O-ring in place. They said it is fine to run without it and most likely when it fell off it just got burned up and sent out the exhaust. Phew!
