Default Voltage & lean issue

Hello everyone hope all is well with you all.

I've posted other threads on here and greatly appreciate all the advice and info.

"Voltage issue"

​​​​​​I have mention this problem before, but hasn't been solved. It's been getting a bit worst since and want to get it figured out.

So the most noticeable symptoms that shows is "voltage dipping" When cold start, my volts will dip to the high 12s but recover back to the low 13s

When I put it in drive or reverse, voltage will drop so bad that it seem like the car is about to stall out.
I do realize that my brand new ac delco alternator squeak like crazy.. Maybe a defect? One of the bearings went bad?

I also noticed that all these symptoms are most active on cold start..
When warm I barely have voltage dips, I can even move through gears without it wanting to stall.

The whole charging system is pretty much new, beside the starter..
I cleaned all the grounds yesterday and that kinda helped.

What you guys think??

Next, the lean condition.
So I realize that my car is running real lean..
My long term fuel trim (Right & left side) is always pegged at 160 through the open loop process until close loop.

When driving my LTFT (Right side) stays at 160, while the left side would drop around 157, and thats part throttle.

At wide open throttle, both side would drop around 145-150
Once I stop and idle it goes right back to 160 (16 cell)

My short term fuel trim hangs pretty high to (right side) at least around 178 at idle, the left side at the same time be around 147

Once I start driving STFT will drop and hang around 123 right side and 134 left side.

I've check for exhaust leak and vacuum leak and couldn't find nothing..
So there a couple of things I noticed through my scanner.

That my IAC count will drop to 0 in idle, But will cruise around 47-50 while driving.

My o2 sensor act weird in open loop even tho the pcm don't use them in this phase.. I know they suppose to still bounce around, not stuck..

It's real weird how sometime it take awhile to switch back into close loop after being driven

​​​​​​The other night code 44 popped up while at a stop, I replaced it the next day with a another ac Delco o2 sensor

But seem to me that wasn't the problem..
I think my catalytic converter is going, cause occasionally I can smell a rotten egg smell, but not so often.

And I can hear this ticking sound coming from the exhaust tips.

​​​​​​I read that 95 lt1 can run lean without throwing a code, correct me if I'm wrong.

What you guys think? Will a bad catalytic converter cause both bank to run lean?

I was also thinking my injector might be bad, but not sure... Just a thought.

​​​​​It is the original Catalytic converter..
140xxx miles on it.

I know it's not the maf, cause I just replaced it not to long ago. But I know a bad one can affect both sides.

On the scanner maf range from 9-11

I have a 1995 trans with cc502 cam
K&N cold air intake
BBK 52 throttle
Stock exhaust manifold
Solomon tune

Are the ground straps on your motor to chassis good? That could be the problem with the voltage.

They all look good to me, especially after I cleaned them up.
The voltage gotten a little better

So I know it's been a good while since I been on this thread. I been very busy due to all this pandemic stuff. But hope all is well with you everyone. So the picture is the vacumm hose that connect to the evap solenoid. I broke this when I was taking off throttle body when I first got the car. I put a rubber hose and clamp it temporarily. But forgot all about it. Will this cause my long term fuel trim to be at 160 (both banks)?

If it broke engine side of valve, would be a vac leak no?

I'll double check it later.
The other day while driving the the evap duty was showing numbers. Like 22, 33, 44 up until it got to 100

And if I'm not mistaken your not supposed to see those numbers unless there a leak.

It only should show 0-100 when stepping on the gas since that the only time the pcm commands it right?
Correct me if I'm wrong guys.
