Tranny Noises

Tranny Noises I have been noticing a lot of noises coming from my tranny for the last few months. To start, I've got a 13Xk miles on my stock drivetrain/hydraulics (except for 4.10s). No drill mod. I am experiencing a host of problems/symptoms that I want to get some opinions on:
-Sometimes won't go into gear when stopped.
-Sometimes wont go all the way into gear when moving (gotta let the clutch out while keeping pressure on the shifter).
-Occasional squeaking when clutch petal is pressed down.
-Occasional noise (not really grinding, more of a moaning) when shifting into any gear, and sometimes on release of the clutch petal.
-Has felt like the clutch is trying to engage when sitting still with clutch petal in... very rare.
-Clutch fluid turns black pretty fast... I've changed it several times via the "suck fluid out of reservoir, fill it with new, pump petal, repeat as necessary" method.
Now for the good:
I just purchased a Monster Stage III kit with T/O bearing, Slave, clutch, lightened F/W, Pilot, etc. Took advantage of the Holiday Special
I feel like this is a slave/TO Bearing issue.

Right. The clutch still hasnt slipped ever, and I've never taken it to the track. Its pretty much a DD. But you're right, fixing it right away would have been ideal. Its actually only been bad enough to catch my attention enough to prompt me into purchasing a clutch for about 2.5 weeks now. The symptoms arent on every shift either. Probly less than 10% of the shifts. Either way, its time to replace it! 133k is prett good for a stock clutch!
I've got a rattle coming from up front too at about 1600 rpm that is persistant and reliable. Hopefully it is clutch/hydraulic related too!
