Default problems

I was with my friend in his 95 ford ranger and he was accelerating as fast as he could and shifted to second but it made a grinding noise as if he missed but it still went into gear as if it was a clean solid shift....he says it also makes the missed gear grinding noise sometimes when he puts it in reverse any ideas?

It's a worn syncro.Grind it till it fits method of driving.
As for reverse it will do that 'specially a ford, they sometimes have release issues.
