If you're very lucky, you might be able to pull the front cover and get away with installing just a new input shaft. However, price and availability can vary

But yes you also need a new slave too.
And you will need to inspect the clutch, but it is probably ok.

Hole in the bellhousing is not ideal, but if it isnt broken or cracked over the majority of the casting I'd be happy enough re-using it.
