T56 pilot bearing/bushings crushed!!

I have replaced several pilot bearings. They all come out in powder form. I got good and tired of this and thought I might solve the problem by putting in a bushing instead. Ummmm yeah.
I go to pull out the clutch, and see the bushing has suffered extensive damage. It wont even come out and is cracked in 4 or 5 places. I left the damn thing in there because I went TH400. But I would like to know what the hell causes this?!!!
I have never seen the like and I suspect it is part of the reason I kept grenading the T56's. I may want to go back to the T56 later and would like to have the problem solved.

I sure can! I took pics of that gabage! The first time it happened I thought someone pulled a dumbass and just didnt put one in. Then I replaced it. My clutch started slipping that night, so I took it out again only to find the bearing was already crunched up good. I messed with replacing bearings about 5 times. After that I got the bushing. I will post the pic up in a few.

If I had to guess I'd say some possibilities could be missing alignment pins, warped bellhousing, loose bolts on the bellhousing to block or gearbox, bellhousing not being seated all the way, worn input shaft pilot diameter, worn input shaft bearing, or a wobble in the crank (spun bearing, loose main cap etc..), wrong input shaft (myself and someone else on here went through that). Maybe even a badly out of balance clutch disc putting side loads on the input shaft. Looks like the input shaft or crank shaft is wobbling and the input is overheating and smacking against the bearing. Just tossing out some ideas.
