You have a damaged shift fork or worn clutch teeth. This is generally the result of missing a shift and crashing reverse( several times). The cure is a teardown and replacement of linkage, synchrinizer and gear components at the rear of the transmission.

I'm gonna say its the reverse shift fork . Turn the car off , out it into reverse with some force (like you would power shift it racing) . Start the car back up and see if it does it still . If so , you are not getting enough throw from the shifter to the reverse slider . Most of the time its the shift fork and or shift fork pads only if you are lucky .
