Disconnect codes????

Disconnect codes???? About a year ago I rebuilt my engine, and have put over 15,000 problem free miles on the car since. But in the last 5000 miles the car has been acting really weird. At first it was just when I hit a bump on the highway, the car would stutter, basically shut off then as the clutch was already engaged and the car was moving the car would start itself back up again. Now when I'm going down the road the car just goes from running fine to 
to shutting down the engine all together. The throttle does nothing. I believe the car is cutting the ignition and the fuel. Then If I wait for about a day or sometimes just a few minutes the car startes right up and runs just fine. These are the codes I got when I scaned the car with efilive scan tool.

This is my daily driver and it last crapped out on my way to the airport and is sitting a parking garage for 22.00 a day until I can figure this out so if you can help I would appreciate it very much.
