Default oxygen sensor keeps going bad

i have had my mods listed below in sig for about 3yrs now. once a year the passenger side 02 will go out. Driver side never goes out. i have a dyno tune too. i get the codes P0151 and P0174. i have a diablo programmer and it shows 0mv to the 02 sensor. i have checked for leaks, maf is clean, new fuel filter, and other then that i dont know what else to check. my oil filler cap dont seal all the way could that be problem? . i have been using bosh 13111 02 sensors. is this right? do i need upstream or down stream 02 for long tubes or do it matter? i looked on forum and some people say use the denso?? any 1 know the part number? also when u put in the 02 sensor, do u need to have it real tight? i tighten about the same as i do the spark plugs.

The sensor is probably not bad, just over-criticized in a bad
environment. If you killed some of the switching / heater
codes you could probably stay with closed loop operation.
If they switch then they're not dead. But they would like
more heat.
