Default Rich / Lean Codes

All my mods are listed with my profile:

I've used a Diablo tuner for the last few years and had it dyno'ed using the same hand held.

To attain a 12.5 to 1 AFR the injector slope setting was at -10% or with the logic it's adding 10% more fuel. Over time I've gotten lean codes and now am set at 18%. I've taken the CAI off, cleaned every item and re-attached including the MAF. The inside of my exhaust tips are black - so I rich.

With spring here I want to resolve this issue: Could I have a bad O2 sensor?
I know I could be pulling air some where but I'm not sure how to test for it.
Would Sea Foam help assist in identifying the leak. I've removed the catch can and am recycling dirty air and nothings changed.
Any thoughts on to resolve would be appreciated.

Those codes don't have to do with WOT, they have to do with
closed loop. The crappy stock WOT mixture is a commanded
AFR problem, and shouldn't be "fixed" by lying about the fuel
delivery. It may be the only tool in that toolbox but you're
basically pounding screws.

Black does not mean rich unless it's rich to the point of misfire.
Every exhaust accumulates carbon.
Rich makes CO. Lean makes NOx. Sweet, or sour smell at idle?

If these new Predators do real time display look at where your
high trims are. Look at the O2 sensor voltages for activity while
in closed loop.
