ltft max bank 1 koeo
I have 94 impala with a lq4 with ls6 intake... running lean bank 1 my ltft are max out with koeo and koer... I don't have exhaust leak or intake manifold leaks.. used a smoke machine... I already swap out maf and o2 still have the same problem.. I thinkinh my harness is jacked up.. with koeo my ltft bank 1 25% bank2 -15

Did you have a scan tool? IF so, unplug the o2 sensor and see if it drops to .450mv. if it doesn't move, then you have the bank 1 sensor 1 wrong. So then unplug the ones until you see bank one sensor one go to .450. then that is the bank 1 sensor 1. As for the other issues of the other bank being really rich, It could be mass air meter placement, or it could be tune related, or it could be fuel pressure not being correct...
There are only 4 wires going to your o2 sensors, the 2 Signal wires and one power and one ground for the heater circuit.. Check to see the pink wire has 12v when your key is on, and the black is ground. Then the 2 left go to the computer triggers for MV signal.
My guess is, that you don't have the right bank 1 sensor 1. that needs to be on the driver side manifold.

Yeah I have seen that a lot, it is pretty easy to get them swapped.
Now, to get the trims figured out, you need to make sure you have clean maf, Try unplugging it and seeing if the trims go closer to 0, IF THEY DONT, then modify fuel pressure to get them closer to 0, then put the maf back on.
