P1515 trouble

P1515 trouble I'm not sure what to do next. I have a lq4 swapped into my 67 Chevy truck. The motor is a 6.0 from a Yukon Denali. I've checked the yello wire at the throttle actuator but looks good. TAC module had a hole in it from before I got it so I found another with same part number on eBay. I changed the throttle position sensor early last summer because it wouldn't shift out of first at WOT. Ran great until a couple weeks ago. Should I replace the throttle or change the actuator motor? I did this swap 3 years ago without any trouble. Thanks for any help.
