Non pre defined wideband setup I NEED HELP

How does one go about setting this up using the latest hptuners? My other wideband is pre defined (ballenger). Im unsure how to set this up. Here is what i have from the wideband manufacturer. i will be logging this through the pro interface.

0-5V analog output scaling formula for Gasoline AFR = (2 * Volts) + 9

The Tuning School put a video out on how to do it but basically just select one of the preset units and transform the functions to match your wideband. Fast forward 4 minutes:
