Default P0606 code after base tune?

I took my gen4 l96 6.0 nova swap to the tuner last week and now getting p0606 "control module internal performance" code. At first it set that code plus p0151 bank 2 low voltage o2 and the p0606. Freezeframe on that showed 0 volts on the o2. I swapped o2's from bank to bank, no more codes on that yet.
The p0606 is intermittent as sometimes will set at key on and sometimes not. The symptoms is that the throttle goes dead and engine idles only after about 70% with load, going up hills, freeway on ramps for example.
I did get the engine running before the base tune and it never set that code. At least not a dtc. I didn't check the freeze frame at that time.
Engine is mostly stock other than Lq9 flattop pistons, cam, valve train, vvt delete, e38 computer, BP standalone harness, t42tcm with 4l60e, headers.
Any ideas would be helpful. I haven't contacted the tuner yet, but I will.
thanks

Did the subsequent P0606 move to the other bank?

The 606 code is usually the ECM death code. Its from cross flashing the wrong OS in the ECM usually. I have seen a few times when the fueling was just that far off it throws that code. We have had to replace 90% of the ecm's when a swap comes in with that code. We have seen people try to delete the 606 code in the ecm, it will just go to limp mode or the throttle wont correctly and not throw a code.

Yup. My guess is that the tuner replaced the OS and didn't pay attention to the throttle mapping tables being different between the two. Nobody seems to understand that you can't just change out the OS on E38s and E67s like you could on the Gen3 stuff since the integrated TAC module portion on the Gen4 ECMs isn't re-written with HPT, etc when you do a full write entire. You have to use an MDI for the Gen4 stuff to change operating systems and then you have all sorts of service/hardware IDs that are compatible with certain operating systems/years.

So the ecm might be able to be repaired by reflashing the correct gen4 os?

Could having the the wrong Accelerator Pedal cause the p0606? I'm using ctsv assembly.

So the tuner said he never touches throttle mapping because he is aware that can cause the p0606.
Any other ideas of how to track down this code problem?

Update. I sent the ecm to BP automotive. They were able to fix throttle tables. One more problem off the check list. Now intermittent no 4th gear up shift with no codes.
