How much have you opened the throttle blade up?
It needs more air. I don't think I've ever drilled a bigger hole in a T/B.

Bank 1 oxygen sensor is dead lean at idle and moves when you snap the throttle, so that one appears to be working.
Bank 2 is dead or not hooked up.

You have to open up the TB a LOT, before the voltage gets high enough to throw a DTC.
To start out, open it up 1/2 to 3/4 of a turn, do a TPS reset and see how it does. If it's still struggling. Open it another 1/2 turn and repeat.
Tuning the VE and MAF will normally require you to mess with the screw several times before it's right. Then you have to tune the BRAF after that.
and you need to fix that 02 sensor.
